In the last 14 days:
- I haven’t been diagnosed with confirmed or suspected Covid-19 infection.
- I am not waiting for a test result relative to Covid-19.
- I haven’t travelled abroad.
- I haven’t been advised by the HSE that I have been in close contact with a person who is a confirmed
- or suspected case of Covid-19
- I haven’t been advised by a doctor to self-isolate.
- I haven’t been advised by a doctor to cocoon.
- I don’t have symptoms of cough, fever, high temprature, sore throat, runny nose or flu-like symptoms
