I’m free from Covid-19 symptoms and I’m travelling to (the home club) today.

In the last 14 days:

• I haven’t been diagnosed with confirmed or suspected Covid-19 infection.
• I am not waiting for a test result relative to Covid-19.
• I haven’t travelled abroad.
• I haven’t been advised by the HSE that I have been in close contact with a person who is a confirmed
• or suspected case of Covid-19 

• I haven’t been advised by a doctor to self-isolate.
• I haven’t been advised by a doctor to cocoon.
• I don’t have symptoms of cough, fever, high temprature, sore throat, runny nose or flu-like symptoms
